NO HAZARDOUS MATERIALS in this container

NO MUNICIPAL WASTE in this container

NO BURNING OR HOT ASHES in this container

NO LIQUIDS OR WET CONCRETE in this container

NO TIRES, REFRIGERATORS, AIR CONDITIONERS without prior approval

and arrangement with Galaxy Transfer Systems Inc.

Material in container must be evenly distributed or Rentee is responsible

for the load adjustment. Load MUST not exceed the top of the container.

Load MUST not overhang the width of the container

CUSTOMER AGREES TO PAY ANY OVERWEIGHT CHARGES DUE TO EXCESS

LOADING OF CONTAINER

AT NO TIME SHALL THE CONTAINER EXCEED 20,000 LBS.

Customer is solely  responsible for over weight tickets

issued by an y la w enforcement authority  due to improper loading of the container

CUSTOMER AGREES TO PAY ANY OVERWEIGHT CHARGES IN EXCESS OF THE

ALLOWABLE TONNAGE LISTED ON THE FRONT OF THIS FORM OR FOUR TONS,

WHICHEVER IS THE GREATER AMOUNT

GALAXY TRANSFER SYSTEMS, INC. (GTS) ASSUMES NO LIABILITY FOR ANY

PROPERTY DAMAGE AS A RESULT OF PLACEMENT INSTRUCTIONS BY THE

CUSTOMER, IT’S CONTRACTORS OR REPRESENTATIVES INCLUDING, BUT NOT

LIMITED TO: DRIVEWAYS, CURBS, LANDSCAPING, BUSHES, PAVERS, ETC.
